Abstract Background Penetrating brain injury (PBI) can be caused by several objects ranging from knives to chopsticks. Case Presentation We presented case 26-year-old male alleged history insertion eight nails machine right side head accompanied loss consciousness vomiting. Computer tomography demonstrated that passed through parietal bone reached up contralateral hemisphere posterior cranial fossa. Removal hematoma evacuation was done timely led patient have good postoperative recovery. Conclusion In this report, we discussed successful very rare penetrating machine.
